1. The English version of these terms and conditions shall prevail whenever there is a discrepancy between the English and Chinese versions.
本條款及細則的英文版本如和中文譯本之文義有任何差異,一概以英文版本為準。

2. Company offers no warranty of any kind with respect to the test service provided by to the clients
本公司不會對所提供給客戶之服務提供任何形式的保證。

3. The test service is not intended to provide any medical advice and serves no purpose for diagnosis and or treatment.
測試服務的的並不旨在提供任何醫學建議和作出任何的診斷和/或治療。

4. Scientific research is ongoing and any update and future development for the test is under a separate agreement.
科研正不斷發展中, 若測試項目於將來有進一步更新, 該更新測試將依據另一測試合約條款及細則進行。

5. Test sample will be used for test service provision and internal research and development only. If internal research and development to be conducted, all personal identification will be concealed.
樣本只會用作測試服務及內部研究用途。 如樣本用作研究之用,所有個人身份證明資料將被隱藏。

6. Company reserves the right to amend or withdraw terms or conditions without prior notice.
公司保留對條款或細條之更改或取消之權利, 恕不另行通知。

7. Unless for the reporting on critical result or result associated with public health concern, the company would normally contact you through your referral physician. The company could report test results to relevant regulatory body, if such reporting is required by law.
除非測試結果對病人健康狀況有即時關鍵性影響或涉及公眾健康考量, 公司一般只透過相關轉介醫生和閣下聯絡。如基於法規要求, 公司可能將測試結果匯報至有關監管機構。

8. Company recognizes its responsibilities in relation to the collection, holding, processing, use and / or transfer of personal data under the Personal Data (Privacy) Ordinance (Cap. 486) (“PDPO”). Personal data will be collected only for lawful and relevant purposes and all practicable steps will be taken to ensure that personal data held by the Company is accurate. The Company will take all practicable steps to ensure security of the personal data and to avoid unauthorized or accidental access, erasure or other use.
公司明白其就《個人資料(私隱)條例》(香港法例第486章) (「條例」) 收集、持有、處理、使用/或轉移個人資料所負有的責任。本公司僅將為合法和相關的目的收集個人資料，並將採取一切切實可行的步驟，確保本公司所持有個人資料的安全性，及避免發生未經授權或因意外而擅自取得、刪除或另行使用個人資料的情況。

9. Your personal data will be kept for at least one year. Any request on the erase of personal information must be sent by written notice to the company.
閣下的個人資料將會被保留不少於一年, 如欲刪除個人資料請以書面形式向公司申請。

10. In case of any disputes, the decision of 「Health Hope International Medical Laboratory Limited」shall be served as final.

如有任何爭議, 「希華檢測(國際)有限公司」保留最終之決定權。
